You should be able to have a veterinarian to fill out the Statement of Identification for you--that used to be the alternative.Call USA and ask them what you should do if you can't get the breed warden. I got lucky with my pup, the breeder did the "Statement of Identification" for me before I took him home. I just sent the paperwork in about three weeks ago with his xrays so now I wait.
I'd help (I've been a Breed Warden for about 10 years), but I'm in the wrong part of the country....
