Potty Training: In House During Day

So Willa will be 5 mos on the 11th, and is 100% crate trained, and will hold it through the night with no problem. But she does not understand that she can't pee in the house. I let her out at intervals, and praise and treat, but if she needs to go while inside, she just goes. On the rug, in the kitchen, upstairs...help? We have a carpet shampooed and use it so that it isn't a scent thing. She has recently started pooping inside as well. grrrrrrrrrr

I would go back to the beginning with Willa just like if she were only 8 weeks old. When I take her out of the crate - I would take her outside immediately and give her enough time to go and also walk her. I would not let her loose inside the house but keep her tethered to me so I could catch any accidents.

she can only go in the house when you're not watching her.
take her out often.

Unfortunately, that is not the case, there have been many instances when she walked down right in front of us. We immediately stop her and grab her and take her out, but it doesn't seem to deter from the next time

Would she use it in an x pen? I just started letting Layla loose in the house, she was in the X pen inside if I wasn't directly interacting with her. She never used it in there.
