Help with dog confusion

I am helping a friend, who got a rescued 20 month GSD from the shelter,
Dog have some training, not pulling while walking and respond to some verbal commands.
Problem is with Sit and Down, Doggy seems to be confuse with commands, when command is down, the dog nicely go down, but stay is not clear,
When command is Sit and the dog is coming facing us, the dog heel sitting on the left…
When the dog is “Down” and he is order sit the dog just circle and stay down again with a an expression like “I don’t understand”
I train my dogs first with Sit and then Down which never require for me to teach sit again or in between, so I don’t know how to improve this.
Funny thing is with a bowl of food on hand If the dog is order to sit he sit but starts to give the paw…
Also the dog is kind of obsessive giving the paw for almost anything.
I hope make myself clear.
How can we teach the sit coming from standing or from down?
How can we avoid the “giving the paw” for almost anything?

You don't say what you are doing for rewards. Do you use a marker word? Clicker?
Using a bridge word, like "good down" will keep them in the position until you give the dog the release word. Changing up what you do helps the dog to focus on you, because they don't know what is coming next.
Clicker training is good for holding a dog in the position along with the bridge. They hear the click, and know they can be released, but will wait until they hear that click.
Once the dog is proofed on the commands, having him do it from a sit, stand, down should be automatic.
Ignore the pawing and he'll get the hint-it gets him nothing...
