I switched my puppy to RAW the minute I brought her home from the breeder at 8 weeks. she has never had kibble since she left her mommy and never looked back.
you should not mix kibble an raw in the same meal.
this is her first raw meal ever.

I think mixing it is ok, but the pup will probably ignore the kibble in favor of the raw food. Depends on the food drive of course!
For me, the point of feeding a raw diet was to get the dog off kibble and I always went cold turkey when transitioning to raw. You may see some runny poo during that time, it is normal.

I would feed 3 meals a day 8 am, 2 or 3 pm, and 8 or 9 pm
At about 16 weeks is when I went to two meals, with a midnight snack.

I'm not sure what you are feeding so can't say.
Yes separate bowls and possibly away from each other.

check out this website: www.rawdogranch.com
All the info to get you started is there, especially the spreadsheet.
I started with 2% of my pup's expected adult weight per day. so I am counting on Fanta weighing about 75lbs when grown (based on her mother) therefore 2% was 1.5 pound of food per day. I started with 50% RMB (raw meaty bone) and 50% MM (muscle meat).
