My dogs have them, but neither required surgery. Some do require surgery for medical reasons and others will have it done for cosmetic reasons. I don't think it's too big of a deal though.

Ranger was our first crate trained dog. He was never thrilled about it, but tolerated it ok. It took a while as he could be destructive, but he got his freedom around one year.

Educator/Einstein, Dogtra, Tritronic, are commonly mentioned. We have an Educator ET 300 and a Dogtra 2300, both have worked well for us. You might try asking those you train with what they use.
