Required Inoculations for OB school

Not sure if this is the correct forum but, it is for Obedience school, and this is an obedience forum, so.....
We start obedience school on the 6th, and they sent me a Vet form to fill out for dates of A) rabies, B) dhlpp, C) other, and D) other.
Bear is current on everything, but my question is for dhlpp, the vet records I have states dhlp - cpv #1, dhlp - cpv #2, and dhlp - cpv # 3, I should use the 3rd one's date, right? But, what do I list in the 2 spaces for "other"? I guess I could call or go by the vets, but I felt I have a better informed knowledge base here!

I would not put anything else on there. I guess if Bear has had lyme vaccine or kennel cough you can add that on there but it does not look like the require anything but rabies or the combo vac.

If you do the vaccines separately then you would have to list the most current or recent date for each one. Some vets separate the parvo from the distemper vaccine so they would be given separately.

And if there is a question in your mind bring the shot record to class the first night in case they ask about it.
