A combination of 50% Apple Cider Vinegar and 50% water rinse after his bath with an organic shampoo , as Doggie Dad mentioned, but not with oatmeal as an ingredient, in case he is grain sensitive. Have some treats ready....some dogs do not like the smell of vinegar.
You can also give Benedryl.

Have you considered a raw diet?
I would definitely vet first, though. When you're considering it, the symptoms probably warrant it. Make an appointment
I, personally, probably wouldn't mention PMR (prey model raw) to the vet, though. They're not usually fans. Might put you through several months of a science diet first. Some, not all vets. There are many good threads in the forum, lots of success stories with skin issues resolved with a raw diet. And just as many, or more, science diet reviews.

Dealing with Itching and Scratching?
I've just changed my two-year-old's food. He was on a lamb w/ a chicken stock (Natural Instinct). I changed it to Merrick -- duck and potato, no grain, no chicken stock. Pork stock.
I just changed his food so nothing huge can happen to his itchiness yet. But in the mean time, any suggestions for how to help his skin feel better?
I feel so badly for him watching his itch and scratch.
Should I take him to the vet?
