Default Crusty sores on elbows and hocks. Help!

We have battled crusty red sores on our nine month GSD for several months now. He gets these sores mainly on his elbows, but I sometimes see them on his hocks (or rather thigh area) too. I have tried medicated shampoos on a frequent basis (as recommended per shampoo instructions), but they still manage to keep coming back. It seems to be spreading this time on his one elbow. It looks really sore (red, inflamed) with some black specks in it that look like ingrown hairs or dried blood, not sure? Does anyone have an issue similar? He has been to the vet twice for this and he was on antibiotics. Nothing is helping. Is there anyone that can suggest something we haven't already tried. He has been eating a lamb based grain free food, so I can't imagine it is an allergy.

Yes

Elbows are a pressure point. Does he plop hard when he lays down or settle a lot on a hard surface?

He does spend time in his plastic crate at night since he is not trusted to be left out yet. Other than that he mostly lays around on carpeted flooring throughout the day. I bought some of that spray, so we will see how it goes.
