Juvenil Renal Disease (which is really a misleading name as it is really renal FAILURE) is genetic, and at least in the UK has been seen in some lines of GSD. It is commonly seen with a high BUN, high Creatinine, and protein in the urine. Dilute urine would also be a sign of renal failure.
Unfortunately, like any renal failure case that's not toxin related or other disease process related, there really is no "cure", only managing it. Therefor, not a long prognosis. High levels of fluids - this involves potentially giving fluids under the skin at home every day from the direction of your vet. I would NOT want to wait 6 months for more blood work. This disease develops fast and most of these dogs rarely make it past 2 years. I would probably repeat in a month or so, if values are even worse then treatment options can be discussed.
