Some ears don't come up until a year. Even if they come up now, they'll be down when the puppy starts teething. Start worrying at 4.5 month old

Isn't 4 months about when they start teething? I'd think once they are through teething (around 7'ish months?) is when you might know if they are going to come up. But even then, I've read some threads on here where they haven't come up until a year or later.

Every GSD is going to be on a different schedule.

That's still pretty young. You can always glue to help it come up.

He is still pretty young. I wouldn't worry about it. Below are some things you can try.

*Feed RAW bones: there are over 150 muscles in the jaw that help strengthen the ears with all the tugging & chewing. Ask the butcher to save you the large knuckle bones. Start out giving the bone for a short amount of time once/twice per day as it can cause diarrhea in some dogs. Put bone in frig or freezer in-between times to keep it fresh.

*Ear exercises: Unexpectedly, whistle, squeak a toy, or call his name many times during the day to make his ears stand for a second!

*Gently pinch the base of the ears together and massage the base without bending the upper ear.

*Dog Crates: Sometimes if a crate is too small and the dogs head is touching the top of the crate, his ears are being forced down, not allowing them to stand.

4.5 is when I would start helping, basically after its done teething, that's when I'd help.

Give him plenty of things to chew on to help build up the cartilage in the ears. I used a kong, and antler bones after soaking it in chicken broth. The more they chew the stronger the Cartilage will be and he won't chew up any of your belongings.
