Starting out on a raw diet

I have a 6 month old shepherd puppy, he weighs about 68 lbs and I want to start feeding him a 100% raw diet. How do I start him off, just one day feed him completly raw or slowly transition him from dry kibble to raw. I have read that you should start him on one source (like chicken) of protein for a week and then introduce something else. Can someone please give me a sample menu for a day of feeding. Also should he be eating anything besides meat to get all the nutrition he needs? I am kind of scared to give him whole chicken bones. Can i grind up the meat and bones and would it still be the same as feeding it to him whole? Thanks in advance for the advice.
