(Sorry cat stepped on the keyboard while I was typing.) If the odor is that bad, personally I would be heading to the vet. With him scratching a lot, the scratches in his ears and his bad breath it could be many of the things they are saying online. It sounds like an infection if the smell is that bad. Best to take him to the vet. Plus any time there is labored breathing, personally I would think you are past the "trying to do anything before going to the vet".
